ASBAIT/Medical POS II – District Paid This plan, provides access to Banner Health/Aetna Network Provider.

You can find any physician or specialist in the directory under Banner Choice POS II or Aetna POS II Network without a referral here. By receiving your care and services from a provider in the Banner Choice POS II Network, you will receive maximum savings of benefit and therefore have a less out-of-pocket expense. Employee medical benefits begin on the first day of the month following date of hire.

PRESCRIPTIONS – Available through CVS Caremark
The plan can be used at your local pharmacy or you may obtain your prescriptions by mail order. Mandatory generic is a $15 co-pay. Brand (when no generic available) is 20% co-pay (minimum $20- maximum $50). Brand (when generic is available) is 20% co-pay + cost between generic and brand. Mail order for mandatory generic (90 day supply) is a $30 co-pay. Mail order Brand (when no generic available) is 20% co-pay (minimum $40-maximum $100). Mail order (when generic is available) is 20% co-pay + cost between generic and brand.

ARIZONA STATE RETIREMENT SYSTEM (ASRS)

Arizona State Retirement System (ASRS) contribution for 2021-2022 is 12.25% of wages. This program is designed to provide supplemental income to you and your family in the event of retirement or disability. The amount of contribution is shared equally by you and the Buckeye Elementary School District. SUPPLEMENTAL COVERAGE PLANS

FLEXIBLE SPENDING ACCOUNT (FSA)

The voluntary FSA plan is serviced by Meritain Health. You can enroll in this benefit if you have the Silver or Gold plan. A $5.00 monthly fee will be collected through payroll deductions for this service.

Use pre-tax dollars to pay for eligible health care expenses not reimbursed by an insurance plan. All IRS code 213(d) expenses are eligible, including your deductible, coinsurance and copays, and expenses above usual and customary limits. Out-of-pocket expenses on prescription drugs, dental, vision, hearing and orthodontic care are eligible as well. Certain over-the-counter items may qualify, too. {Dependent care costs. Pre-tax dollars can be set aside for day care type expenses for eligible children or adults. Expenses are eligible if they’re for the care of a person under age 13, or an older dependent who is unable to care for themselves. They must regularly spend at least eight hours a day in your home.

HEALTH SAVING ACCOUNT (HSA)

Health Savings Account allows you to contribute pre-tax dollars for you to pay for out-of-pocket medical expenses, such as co-pays, deductibles, prescriptions, glasses, dental, and other related medical expenses. Buckeye Elementary School District will be contributing $1,422.00 to the employees Health Savings Account, effective 7/1/2022. You may also contribute to your account based on the coverage you select.

$3,650 Individual contributions

$7,300 Family contribution

$1,000 catch-up contributions for members over 55 years
